The more we know about your ward, the greater the attention we can give and the more
fun we can have. Fill up the enrollment form carefully and mention all health related
problems. State all allergies. State the anti-allergen that the child normally takes. Make
two tablets of such medication available to the program. Activities may be travel based.
Inform us if the child is prone to motion sickness. |

Write down specific situations and material that can overly excite your ward or make
him/her uncomfortable: for example the presence of milk in food (?). |

|
| » |
Mention specific behavioral issues that educators need to be aware of. |
| |
|
| » |
Make sure that someone responsible is accessible on the phone (cell/mobile, preferably
landline) during the complete hours that the participant is in our care. |

In case of special circumstances, such as asthma, please make sure that the program is
made aware as to where you or your child place supporting material such as inhalers. |

Important: no child is allowed to call other children names or kick, punch, beat them. It
is the parent/guardians’ responsibility to assure that your ward is well-briefed on this
end. |
